New patient paperwork is the same at every Morrison Dental office: a full review of your medical and dental history so your first evaluation is thorough, not rushed. Where you go to fill it out depends on which office you're seeing, since Bluffton runs on a different system than the Georgia offices. Everything on this page is the real, current way to get it done, no online scheduler required, just a form, a call, or both.
Your First Visit
What actually happens between booking and sitting in the chair
Nothing complicated, just three steps, in this order.
Fill out your form before you arrive
Complete your new patient form online, or print the paper version and bring it filled out. Either way, we review your full medical and dental history before you're in the chair, so the doctor already knows your history instead of learning it during the appointment.
Bring your insurance card and ID
Bring your dental insurance card, a photo ID, and a list of any medications you're currently taking. If you'd rather not fill anything out ahead of time, call your office and ask for the paper forms to be mailed or faxed to you.
Get a complete, unhurried evaluation
Your first appointment is a full dental evaluation, not just a quick look. We go through your history, check for anything that needs attention now versus later, and talk through a plan with you before any treatment starts.

Two form systems, because Bluffton runs on its own
Here's the useful part most sites bury: Savannah (both offices) and Brunswick share one online form system, the Adult Patient Form and Child Patient Form. Bluffton uses a separate secure patient portal, forms.patientconnect365.com, because the Bluffton office runs on different scheduling and records software than the Georgia offices. Neither is better, they're just different systems, so use the one that matches the office you're actually visiting.
Savannah, Abercorn Street
Adult Patient Form & Child Patient Form
Complete the Adult Patient Form for anyone 18 and older, or the Child Patient Form for a minor. Both are filled out online before your visit, or printed and brought in with you.
Savannah, Wilmington Island
Adult Patient Form & Child Patient Form
Same two forms as the Abercorn Street office. Complete the one that fits, online or on paper.
Brunswick
Adult Patient Form, Child Patient Form & treatment-specific consents
Standard Adult and Child Patient Forms, plus separate pre- and post-treatment instructions and informed consent forms for Botox, dermal fillers, and trigger point therapy for patients having those procedures done.
Bluffton
Bluffton Patient Portal (forms.patientconnect365.com)
The Bluffton office does not use the Adult or Child Patient Form that the Georgia offices use. New patient paperwork here is completed through a separate portal at forms.patientconnect365.com. Not sure which form or portal applies to you? Send us a message and we'll point you to the right one.
Prefer paper?
Printable versions are available for the Child Patient Form, Adult Patient Form, Temporary Guardian Form, HIPAA Consent Form, Insurance Carrier Information sheet, and our Notice of Privacy Practices. What we accept, and where it's different in Bluffton
We accept many dental insurance plans and file the claim for you, though we don't accept capitation plans, HMOs, or DMOs. Bluffton's accepted-plan list runs separately from the Georgia offices, so call (843) 706-2146 to check your specific plan before your visit.

Big treatment? Spread it into monthly payments
At our Georgia offices, a bigger treatment doesn't have to be paid all at once. CareCredit lets you start treatment right away and pay it off in low monthly payments, financing up to 100% of the cost. The Bluffton office doesn't offer CareCredit. Bluffton payments instead run through a separate payment system called BlueYonder, so ask the Bluffton front desk about payment options at (843) 706-2146.
Medicaid and PeachCare for Kids
Our Georgia offices accept Medicaid and PeachCare for Kids. The Bluffton office does not accept Medicaid or PeachCare for Kids, so South Carolina patients on those plans should call ahead to talk through other options.

What should I bring to my first visit?
Bring your dental insurance card, a photo ID, and a list of any medications you're taking. If you completed your new patient form online ahead of time, you don't need to bring a paper copy. If you'd rather fill it out on paper, arrive a few minutes early or call ahead and we'll have it ready.
Which new patient form do I need?
If you're seeing us in Savannah (Abercorn Street or Wilmington Island) or Brunswick, you'll fill out the Adult Patient Form or Child Patient Form. If you're seeing us in Bluffton, you'll use the separate Bluffton patient portal at forms.patientconnect365.com instead, since that office runs on its own system. At our Georgia offices, yes, through CareCredit, which lets you start treatment right away and pay it off in low monthly payments. The Bluffton office doesn't offer CareCredit and instead handles payments through a separate system called BlueYonder. Call the office you're visiting and we'll walk you through what's available.
What if I have a dental emergency?

Do you see kids, or just adults?
We see patients from infancy through adulthood. For a child's first visit, fill out the Child Patient Form (online for the Georgia offices, or through the Bluffton portal) instead of the Adult Patient Form, and bring the same insurance card and ID you'd bring for anyone else in the family.
